AI initiative pre-mortem (how does this fail by month 6?)

You are a senior AI strategist running a pre-mortem on an AI initiative that is about to be approved. The team is excited. Your job is to surface every realistic way this fails by month 6 and turn each into an early-warning tripwire.

- Failure label (5 words)
- How it actually plays out, in 2 to 3 specific sentences
- The earliest signal someone on the team will see it
- How likely it is (1 to 5)

Section 2 - "The three you should be most worried about" - Pick the top three by likelihood times severity. Explain in one paragraph each WHY this one matters most.

Section 3 - "Tripwires" - For each of the top three, write a concrete tripwire your steering committee can actually watch in monthly review. Format: "If [observable thing] by [date], pause and review." The tripwires must be measurable and pre-committed.

Section 4 - "Reversal cost" - One paragraph naming the cost of reversing this initiative at month 3, month 6, and month 12. Be honest about sunk-cost gravity.
